Most cases are answered inside the $129. If yours turns out to need much more than that, a long clinical history worked through or imaging studies reviewed in depth, I tell you what the extra costs before I do any of it, and you pay that on the payment page only if you want to go ahead. If I can answer it for the $129, I just answer it.
Because you should be able to see what this costs without an email exchange first. $129 gets your case read and answered by a surgeon. If a case genuinely needs far more work than that, I quote the extra before doing it rather than building the possibility into everyone else's price.
