Because cases vary enormously: one clear question with a short history is a different job from years of records, multiple imaging studies and a failed surgery. The range keeps simple cases cheap; the fixed fee before payment keeps complex ones honest.
If I take a case and cannot deliver a useful opinion, you get your money back. If I can see up front that it is not a case for me, I tell you before you pay anything.
Some policies cover specialist consultation fees, and a written surgical opinion can also support claims about when a condition began. Check your policy wording; the itemised invoice I issue is written with claims in mind.
